New patients please complete the following:
- Please read Notice of Health
updated July 1st, 2010, and sign the
Receipt of Privacy Practicies.
- Please Register and provide Contact Information.
- Complete an Adult History Form page 1 and page 2.
- Learn and acknowledge our Cancellation / No-show Policy.
- Inform us regarding who can access your records on our Confidentiality Questionnaire.
- If you are transferring your care to us from another office, please complete the Transfer of Medical Records form so that we can request your existing medical records.
